At-Need Checklist

We will need First, Middle, Last (& Maiden) Names of Decedent

1) Will need to bring a copy of the decedent SS Card or Document with number on it to process Social Security Notification & Death Certificate

2) Important information on Death Certificates Needed to process:

 A) DOB

 B) Birthplace (Co/State or Foreign Country)

C)Usual Occupation (Cannot use “Retired”)

D)Kind of Business/Industry

E) Fathers – First, Middle & Last Name

F) Mothers First, Middle & (Prior name to first marriage-maiden name)

3) Military Honors – Will need a DD214 to process a flag & honors.

** Death Certificates may take up two 2 weeks or longer depending on doctor, pending, etc. The contact person will be contacted as soon as we have possession of them. **
